2) Temporary Food Service Establishment:
A) Establishment Operates 1-3 days $40.00
B) Establishment Operates 4–14 days $60.00
3) Mobile Units $60.00
4) Retail Food Stores:
A) Up to 3000 square feet of floor space. $40.00
B) 3,000-20,000 sq. ft. of floor space $60.00
C) Over 20,000 sq. ft. of floor space. $110.00
5) Carry-out Establishment $60.00
6) Bed & Breakfast Establishment $25.00
7) Locker Plant $25.00
8) Limited Service $25.00
