Walk-In Clinics
      Walk-in clinics operate on a first come first served basis. They are limited to the number of children that can be served by the available nursing staff.

Appointments
    
     We have appointments available at all three clinic locations. To schedule an appointment, feel free to call (260) 499-4182 ext 228. The LaGrange Main office is closed Thursday for off site clinics.
Cancellations
     Clinic days and times are subject to change due to illness, weather and staff in-services. If time allows, clinic changes will be reported on Thunder County Radio 105.5. If school is cancelled due to weather, immunization clinics will be closed.

 

Clinic Requirements

     To receive immunizations you must:


               1. Be the child’s parent or have proof that you are the child’s legal guardian OR You are at least 18 years old and have a note from the child’s parent or legal guardian, giving you permission to bring the child. A note is needed for every visit.


               2. Permission forms must be completed each time you visit the Clinic.


               3. Child’s immunization history is required every visit.


               4. Child must not have a croupy cough, fever, vomiting, or diarrhea in the past 24 hours.


Tuberculosis Test (TB test)


      The LaGrange County Health Department offers tuberculosis (TB) skin testing on Monday from 8:30 am to 3:30 pm at the main office. Cost is $10.00 for the test, and it's read on Wednesday. 

 

The nursing office provides extensive case management services to residents diagnosed with active or latent TB. These services include but are not limited to free medications and direct observed therapy.
